This form is for professionals involved in the provision of care to individuals who may have a problem with alcohol.  It is intended for use by organisations which have an agreed referral pathway with Action for Change.  If you require further advice on the use of this referral form please contact your nearest Action for Change office.

Please complete the form in as much detail as possible and either post to your nearest Action for Change Office, marked clearly as ‘Strictly Confidential’  or hand to your designated Action for Change Alcohol Worker.
